This waiver service provides one-on-one support for children and adults with mental retardation, co-existing mental health disabilities, or autism. Individuals served through this program usually live at home with family members, in personal care homes, shelters, or independently in the community. The EIHAB staff members who provide this service are trained to address the individual’s ISP and its implementation, leading toward greater independence. They are also adept at incident reporting protocol, and abuse and neglect prevention. Daily progress notes are sent to the participant’s support coordinator. Habilitation counselors participate in ISP reviews, and attend day service, supportive employment, and school-based progress reviews.
