Requirements
Masters Degree in psychology or related field, 5-7 years experience dealing with developmentally disabled population. Must possess excellent writing and organizational skills, requires strong interpersonal and communication skills; ability to work well under pressure; willing to help others when crisis arises. The Direct Care Counselor requires specialized training to work with individuals with developmental disabilities in residential living arrangements. The employee in this position is responsible for the security, safety, and well being of the individuals living within the residence according to his/her specific needs and interests, with the purpose of increasing the quality of life for each person by promoting their respective level of independence.

The function of the Social Worker is to provide direct treatment and case management services to client through individual, group, and family contact, in the Adminissions Intake and Diagnostic and Evaluation Program. The Social Worker assists other members of the treatment team in the development and implementation of a client's treatment plan. The Social Worker is responsible for the acute and thorough documentation of all individual, agency and family contact, court hearings and other agency related process, transportation and visitation arrangements, and other case management related issues. Lastly, the Social Worker involves families in promoting clients treatment plan and serves as a liaison between agency workers, families and clients to ensure the provision of all services recommended in the treatment plan.
Requirements
A Bachelor's degree in a recognized human service discipline including Social Work, Psychology, Human Services, Human Development, or Education from an accredited University. Perform assessments in order to determine the individual’s goals. Supervise Com-Hab staff and handle all sensitive issues. Responsible for contacting parents with regard to individual’s progress. Tracking daily attendance. Communicating program information with families and Com-Hab staff on a regular basis as well as the Director of the Department. Ensure that the Director of the Department is notified immediately on all issues arising from the operations of the program. Schedule individual’s visits a week in advanced and post them in the weekly schedule book. Obtain questionnaire surveys from individual’s families on a yearly basis. Compile data on program for individual’s progress and billing purposes (daily/weekly notes) Conduct monthly visits to the individual’s residence to ensure that the staff is performing their responsibilities in line with the goals established. Handle other related duties as assigned by the Director Participate in regular scheduled meetings as assigned by the Director. Train new/existing employees in writing goal sheets. Conduct investigations of incidents reportable/non-reportable. Complete individual’s documentation service (Community-Hab Plans on a monthly basis) Complete Methodology chart, and monthly notes. Responsible for obtaining individual’s documentation from MSC’s (Psychological, Medical, PPD’s Psycho, LoC, DDP, ISP’s, other).
Requirements
Bachelors Degree preferred in a Human Services related field; Must possess an un-restricted New York State Driver’s License. At least one year prior experience working with the mentally retarded/developmentally disabled. Leadership experience including supervising others, program management and experience documenting individual progress; Have some skills in community inclusion and excellent verbal and written communication skills.
